Student Planned Absences

The Planned Absence Form should be used for any student absence that will be one day or more that is planned in advance - this wouldn't be used for illness.  

 

Please print, complete and bring the form to the office for an Administrator's signature and then submit to the School Secretary - this information is then entered into our Student System so that the student absence is excused for the duration of the planned absence.
